Certificate of Insurance

When the University enters into a contract for goods or services, evidence of insurance is often required.  The most common coverages required are General Liability, Automobile Liability, Professional Liability, Commercial Property, and Workers' Compensation. Please complete a Certificate of Insurance Request Form and submit to the Office of Risk Management by fax (612-625-7384) or e-mail (orm@umn.edu).  

The Certificate of Insurance Request form requires the following information:

• Name and address of the organization requiring the certificate
• Type of insurance and limits required (a copy of the insurance portion of the agreement may be submitted)
• Date of the event
• Contact person for the request (phone number and fax number)
