Forms (aka BPEL)

What form(s) do I fill out to request access to initiate requests in BPEL form for Deptid, Program, or CF1?

Go to www1.umn.edu/datasec/security/Requestingaccess.html and select either the "Go to Word" or PDF formatted forms link (whichever is your preference). Find the EFS Data Change Forms (BPEL) attachment. Complete the attachment along with an Access Request Form (ARF).
